How to prepare for the first visit
First:
We ask that you be prepared for your first visit by printing the patient information form and the medical history form available in this section. Being prepared and having completed these forms prior to your visit will reduce your wait time.
Second:
You must bring the following information with you.
* Drivers License or Photo ID card
* Insurance Card, Medicare Card, Medicaid Card.
* If your insurance company requires a referral for your visit we ask that you have it with you
the day of your appointment.
*Co-payment and/or deductible for your visit, all payments will be due at the time of your visit
*Referral form from a referring doctor (if you were referred by another doctor please bring the
packet and or referral form with you)
Third:
If you have an injury or you are scheduled for testing you may want to have someone available to drive you to and from your appointment.
